About the Summit
​
This Summit brings together all 58 Ontario Health Teams, Primary Care Networks, Patient, Family, and Caregiver Advisors, and health and social partners across the continuum of care to learn, collaborate, and drive real change in integrated healthcare.
Building on the momentum of the inaugural ICAS 2025, this year's theme, Building a System of Care, highlights that transformation is bigger than any single organization. Better health outcomes require moving beyond organizational boundaries – where health and social organizations work collectively, share responsibility for outcomes, and align their efforts to create a more connected, coordinated, and community-driven system of care.
The two-day Summit is a collective space for participants to connect with one another, learn from each other, and expand your collective impact.​

ICAS 2026 will take place at the Hilton Toronto/Markham Suites Conference Centre & Spa. The Planning team has secured a room block at a preferred rate for attendees that require overnight accommodations.

Building a System of Care
Since ICAS 2025, teams have strengthened partnerships, advanced community‑based models, and worked across organizations to better connect care around people and populations. What is emerging now is a broader shift: from delivering integrated initiatives to building a coordinated system of care.
2026 Summit Objectives:
The 2026 Summit will focus on the following objectives:
Share and scale innovation
Highlight locally driven,
co-designed solutions and create space for OHTs, PCNs, and partners to share lessons learned and promising practices that can be implemented and sustained across communities.
Strengthen connection and shared purpose across all OHTs
Build trust, relationships, and collaboration among OHTs, PCNs, and partners to support collective action and show the value and impact of OHTs.
Advance provincial priorities through local action
Support provincial priorities by showcasing how OHTs, PCNs, and partners are working together across the continuum of care to deliver more connected, equitable, and person-centred care.
Facilitate learning that reflects diverse experiences and perspectives
Ensure Summit programming, design, and outputs are informed by the broad range of priorities, perspectives, and lived experiences represented across OHTs, PCNs, and partners.
